Registration checklist … you will need to provide:

1.     Registration form - select the appropriate form:

	Registration form for U6-U12 Recreation (born 8/1/98 - 7/31/06)
	Registration form for U14-U19 Recreation (born 8/1/91 - 7/31/98)         and all Jaws and Sharks Advanced Team Players         (Print using Fit to Printable Area so signatures lines are included)

2.     Official birth certificate (photocopy) required for NEW PLAYERS ONLY.

Players must be age 4 or older on July 31, 2010 for Under 6.

3.     1” x 1” Photo (of head) for Under-12 players and older (ALL players age 10 & older)

4.     Registration fee with cash or check payable to SCCYSC.

                          
Please consider making a donation of any size to our matching grant fund – our goal is to raise $5000 this spring to further support coach training and equipment. We also have families in need of financial assistance – you can sponsor a player for $50. Add your donation to your registration fees and include in the same check.  Thank you!

Financial aid is available for families who cannot pay the entire fee. Applications are available at online or at walk-in registration. We want every child to play who wants to play.

Important information:

•       Sponsoring a team is a great way for a business or individual to support soccer and get your name into the community. Sponsor an entire team with a $300 tax-deductible donation and have your name printed on the team uniforms if payment is received before August 15. The sponsor form and info are at www.santcruzsoccer.com.

•       Volunteer as a coach, assistant coach, team parent, or board member. Check a volunteer box on the registration form, or contact the club by phone or email. Referees and Field Set-Up Staff are PAID positions. Contact the club by phone or email.

•       Refund requests must be made in writing (with a self-addressed, stamped envelope) mailed to: SCCYSC, P.O. Box 2039, Santa Cruz, CA  95063. Requests postmarked before the first scheduled game in September will receive a full refund less club fee. No refunds will be granted after the first game.

•       New this year: the SCCYSC will independently register and insure our U6, U8, U10, and U12 recreation teams.  Therefore, players in these recreation age groups are no longer insured through nor affiliated with the California Youth Soccer Association (CYSA).
