Registration checklist É you will need to provide:

 

1.     Completed Registration Form.  Print using "Fit to Printable Area" or similar to make sure you have a signature line at the bottom of the page.
Please send in TWO copies of the registration form.  Sign both forms in colored ink.

2.     New Players only: Copy of official birth certificate required
Players must be age 4 or older on July 31, 2008 to play in Under 6.

3.     Registration fee with check payable to SCCYSC.
(Mail-in registrations must be postmarked by April 5 to qualify for the early registration fee.)     

April 6-16:

4.   If you want to coach a team: download the Coach Disclosure Form and mail in with your playerÕs
registration form and check the coach box on the registration form.

Important information:

 

*       Sponsoring a team is a great way for a business or individual to support soccer and get your name out into the community. You can sponsor an entire team with a $300 tax-deductible donation ($270 if payment received before May 1). Your name will be printed on all the team's uniforms if payment is received before July 15. If you are interested, click the Sponsor link at the left. Every team needs a sponsor!

*       Referees and Field Set-Up Staff are PAID positions. Contact the club by phone or email.

*       Volunteer as a coach, assistant coach, team parent, or board member. Check a volunteer box on the registration form, or email tony at santacruzsoccer dot com

*       Shoe exchange / equipment donations at walk-in registration. We collect old shoes for kids who need them.

*      Financial aid is available for families who cannot afford to pay the entire fee. Financial aid forms are available here. We want every child to play who wants to play!

*       Refund requests must be made in writing (with a self-addressed, stamped envelope) mailed to: SCCYSC, P.O. Box 2039, Santa Cruz, CA  95063. Requests postmarked by August 30 will receive a refund less the league fees. No refunds will be granted after the first game.

*       Our clubÕs geographic region includes the school districts of the city of Santa Cruz, Bonny Doon, Davenport, and Happy Valley, and all public and private schools within these areas. If you live in other areas of SC County, you may visit the county web site www.sccysl.org for the club in your area.
